1. Print the Speaker Feedback form.

2. Discard the second page (it is part of the web page but not part of the form).

3. Photocopy as many copies of the first page as you need.

Ballots for Ribbons Awarded at Meetings

1. Print the ballots.

2. Discard the second page (it is part of the web page but not part of the ballots).

3. Photocopy as many copies of the first page as you need.

4. Cut the ballots apart on the heavy black lines (the lines only look black on the printed page, not on the web page).

Judges' Scorecards & Ballots

Evaluation Contest

1. Print the Judge's Scorecard & Ballot.

2. Discard the second page (it is part of the web page but not part of the scorecard and ballot).

3. Photocopy as many copies of the first page as you need.

4. After voting, each judge signs his or her ballot, detaches it from the scorecard, and gives it to the Ballot Counter.
